Position Summary As the Registered Nurse RN Case Manager Care Coordination, you will provide comprehensive care coordination for patients, conduct initial assessments, participate in multidisciplinary rounds, communicate discharge plans, and delegate tasks as needed. You will be accountable for the clinical quality of Care Coordination services, identify and resolve barriers to effective patient care, and empower patients and families to participate fully in discharge planning. You will work at Banner – University Medical Center Tucson, a nationally recognized Level 1 Trauma Center and Magnet®-recognized academic medical center.
Core Functions
Manage individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
Act in a leadership role for process improvement activities across patient populations.
Collaboratively develop and manage interdisciplinary patient discharge plans; communicate plans across the continuum of care.
Maintain knowledge of Medicare, Medicaid and other program benefits; assist patients with discharge planning and access to community resources.
Establish collaborative relationships with physicians, payers, and other health‑care team members; collect and communicate information to fulfill utilization and regulatory requirements.
Educate internal team members on case management and managed care concepts; facilitate integration into daily practice.
May supervise other staff.
Determine how to accomplish functions within established procedures; consult supervisor on unusual situations. Positions are entity based with no budgetary responsibility.
Minimum Qualifications
Bachelor’s degree in case management or health care (or equivalent experience).
Current registered nurse (R.N.) license in the state of Arizona.
Basic Life Support (BLS) certification required for acute‑care assignments.
2–3 years of clinical experience, with critical thinking, problem‑solving, communication, and time‑management skills.
Ability to work effectively in an interdisciplinary team format.
For case management positions in acute facilities, a minimum of one year of case‑manager experience in an acute‑care hospital.
Preferred Qualifications
Certification as CCM (Certified Case Manager).
