SCOPE OF ROLE A Licensed Practical Nurse is a medical professional who plans and manages client care according to each individual's needs. They interview clients and record their medical history and physical conditions like blood pressure or temperature while obtaining vital signs. LPN observes patients' health, medical and physical history. LPN on ACT Teams support the Registered nurse with field visits, administering injections, medical education, delivery of oral medications and monitoring medication room. The LPN assesses, monitors, and ensures access to coordinated medical and mental health services as part of an Assertive Community Treatment team.
ESSENTIAL DUTIES & RESPONSIBILITIES Clinical Skills
Administer intramuscular injections and/or supply medications to consumers as prescribed by psychiatrist.
Meet with all consumers in the community on an ongoing basis to monitor health and to monitor and assess consumer progression/regression in behavior and/or appearance.
Meet with consumers with unstable medical or mental health status more often.
Work with consumers in filling prescriptions and coordinating appointments as needed.
Refer consumers to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.
Meeting with consumers both in person and via telehealth visits.
Provide feedback and support to staff during morning meeting regarding clients needs and issues.
Available by cell phone for emergency consultation/crisis intervention
Data and Accountability
Maintain nursing and health related data entry into EHR
Assess medical care needs and other services to support wellness
Act as advocate and liaison for recipients in securing medical care, and other community supports that promote integrated physical and mental health wellness
Review and audit medication management procedures
Administer IM’s.
Transcribe orders and updates Medication Administration Records
Monitor vital signs and side effects of medications and reports findings to Team
Contribute to effective medication management procedures with the RN and program psychiatrist
Assist the RN and psychiatrist in the coordination of medical care
Support the RN to ensure all consumers have primary care providers and receive annual physical and dental examinations
Provide individual health counseling as needed
Compliance
Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
Maintain CPR/First Aid Certification.
Maintain up to date, accurate, complete, and timely consumer chart documentation.
Participates in team meetings, in-service trainings, and in compliance with all online required trainings.
Maintains good status in regard to completion of in-service and online trainings.
Operations
Assist full time Registered Nurse
Record keeping as per agency regulations
Copes adequately with stress on the job.
Applies time management skills to work effectively within given deadlines. Organizes own time and prioritizes workload effectively. Uses resources wisely.
External Relations
Maintain relationships with outpatient programs, housing programs, as well as continuing to build relationships with organizations that may be able to assist SUS consumers who need other services.
Performs other related work as assigned.
Qualifications REQUIRED EDUCATION AND EXPERIENCE
Bachelor Degree or Associates in Nursing, NY Licensed
Experience in the Mental Health service field preferred
PREFERRED QUALIFICATIONS & SKILLS
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
Familiarity with hospital health, safety and sanitation standards and procedures
Understanding of confidentiality obligations and nursing best practices
Excellent knowledge of medical and hospital terminology
Good knowledge of MS Office and data entry
Perfect physical condition and stamina
Excellent communication and interpersonal skills
Compassionate and able to handle stress
Successful completion of LPN license program is a must
