Position Overview This is a 75% field‑based position covering the Lake Zurich/Barrington, IL area. Schedule: Monday through Friday, 8:00 a.m. – 4:30 p.m.
Position Purpose:
Assist in developing, assessing, and coordinating holistic care management activities to enable quality, cost‑effective healthcare outcomes. Help develop personalized service care plans for long‑term care members and educate members and their families/caregivers on available services and benefits.
Responsibilities
Evaluate member needs, available resources, and recommend or facilitate the best plan.
Assist with developing ongoing long‑term care plans and identify required providers, specialists, or community resources.
Coordinate between members/families and care provider teams to ensure services are accessible.
Provide resource support for employment, housing, independent living, justice, foster care, etc., based on assessment and plans.
Monitor care plans, member status, and outcomes; provide recommendations based on needs.
Interact with long‑term care providers and partners to meet member needs.
Collect, document, and maintain member information and care management activities for compliance with state, federal, and third‑party payer regulations.
Perform on‑site visits to assess needs and collaborate with providers or resources as appropriate.
Facilitate education on procedures, provider instructions, service options, referrals, and benefits.
Provide feedback to leadership on opportunities to improve and enhance quality of care and service delivery cost‑effectively.
Perform other duties as assigned.
Comply with all policies and standards.
Education / Experience Requires a Bachelor’s degree and 1 year of related experience, or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope, and skill.
For Illinois, candidates must meet one of three criteria:
RN licensed in Illinois.
Bachelor or Master’s Degree in a human services related field (e.g., Child, Family and Community Services, Early Childhood Development, Guidance and Counseling, Human Development Counseling, Social Work, etc.).
LPN with at least one year of experience conducting comprehensive assessments and providing formal services for the elderly.
Pay & Benefits Pay Range: $22.94 – $38.79 per hour.
