We are seeking dynamic Skilled Nursing Facility (SNF) Rounder Pool physicians who will be responsible for:
Rounding on Kaiser Permanente (KP) members at assigned contracted SNF’s at least twice weekly to address medical needs and concerns, as well as help with care planning going forward
Assuring that the appropriate level of care is being provided and progress is being made towards goals, in a timely manner
Discussing goals of care, transition planning, and advanced directives/POLST, as appropriate
Attending meetings with SNF staff and KP Care Manager to assist with discussions of ongoing care planning, discharge planning and coordination of care for members
Coordinating appointment for evaluation by KP Specialists as needed for member during their stay at the SNF
Working with KP Care Manager and transitions team to facilitate timely post-discharge follow-up for members after discharge from SNF
Working with KP Quality Resource Management Department as needed to accomplish Regional goals
Establishing and maintaining good working relationships with other KP providers, on‑site KP Care Manager, contracted SNF medical and administrative staff, and transitions team
Assessing alignment of patient care goals with care provided by SNF staff, identify systems barriers, and elevate issues when appropriate
Anticipating and planning for needs between visits, including potential discharge planning
Timely documentation, discharge summaries, medicine reconciliation, etc. in KP HealthConnect
Participating in Departmental meetings
Obtaining and maintaining privileges at all assigned skilled nursing facilities
Ability to work weekends if necessary

Minimum Education And Experience Requirements
Physician must be Board certified or Board eligible with Board certification within 24 months of eligibility
Internist, Family Practitioner, Hospitalist, Geriatrician, or Palliative Care Physician with experience and interest in managing inpatients in a skilled nursing facility
Required Licensure, Certification, Registration Or Designation
Physician must be Board certified or Board eligible with Board certification within 24 months of eligibility
