Join Our Team as a Hospice and Palliative Care Nurse Practitioner
Do you value the time you spend with patients? Are you committed to providing quality, compassionate care? We are looking for nurse practitioners who are dedicated to creating meaningful patient experiences.
As a hospice and palliative nurse practitioner, you will care for seriously ill patients to help alleviate burdensome symptoms and align medical treatments with patients’ goals and values. You will collaborate with a team of like-minded clinicians to deliver the best care possible wherever patients call home. As an active member of the medical community in your territory, you will provide ongoing palliative care and hospice service and education.

Responsibilities
Provide direct patient care in hospice and palliative settings.
Alleviate symptoms and coordinate treatment plans aligned with patient goals.
Collaborate with interdisciplinary teams, families, and community resources.
Maintain accurate documentation and comply with regulatory requirements.
Educate patients, families, and staff about palliative care practices.
Qualifications
Heart to serve patients and a passion for compassionate care.
Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
At least 2 years of experience in chronic disease management or long‑term care within the last five years (hospice experience preferred).
Reliable transportation; ability to sit, stand, bend, lift, and move intermittently, including the capacity to lift at least 80–100 lbs.
