Provide care management services to consumers involved with Montgomery County Mental Health Court (MCMHC)
Minimum Experience:
2 to 4 years’ experience working with individuals with mental illness and/or criminal justice system preferred.
Minimum Education:
High School diploma or equivalent. Bachelor's degree in social work, Counseling, Psychology or closely related field.
Certifications, Licenses:
Must have dependable transportation.
A pre-employment criminal background check is required.
General Position Duties And Responsibilities
Continual assessment of consumers’ needs, and development of individualized treatment plan to ensure needs are met.
Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
Ensure consumers are seen upon arrest and release from incarceration.
Ensure consumers are seen upon discharge from the hospital.
Ensure consumer’s daily psychiatric, medical, and environmental needs are planned for and coordinated.
Ensure efficient transition of consumers when needed within the Agency and the community.
Attend MCMHC on a regular basis and assist consumers with legal obligations by providing support and guidance in completing requirements.
Attend MCMHC staffing meetings to give most recent updates on consumer’s progress in treatment.
Attend MCMHC docket reviews to provide updates on participant’s progress and update the court on any barriers to obtaining recommended services in the community.
Promote a recovery focused and trauma informed approach utilizing evidence practices and approaches.
Provide education and direction to participants, family members, and/other members of participants support system.
Respond in timely and professional manner to referrals and inquiries from Court system and probation.
Respond to urgent/crisis consumer needs as appropriate to position with assistance of supervisor.
Consistently treat consumers and co-workers with respect.
Ensure timely and appropriate documentation as required by the Agency.
Responsible for attending all mandatory in-services, continuing education and annual health requirements.
Comply with all Corporate Compliance policies, practices and procedures. Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director.
Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator.
Will be compliant with the relevant regulations concerning the privacy and security of consumer’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Participate in proactive team efforts to achieve departmental and company goals.
Perform other duties as assigned.
Provide leadership to others through example and sharing of knowledge/skill.
Follow all policies and procedures set by the Agency.
Fiscal Responsibilities This position has no fiscal responsibilities.
County Organization Duties and Responsibilities
Represents the Department and County to other County staff and members of the public.
Provides quality customer service to individuals needing directions or assistance.
Demonstrates the organization’s Core Values and adherence to the Ethics policy.
Supervisory Relationships This position has no supervisory responsibilities.

Pertinent Federal, State, and local laws, codes, and regulations concerning process and procedures.
Current office practices and procedures.
Computer software consistent for this position.
Skill in:
Analyzing complex administrative information and issues, defining problems and evaluating alternatives, and recommending methods, procedures, and techniques for resolution of issues.
Organizational and time management to prioritize duties to accomplish a work product while adapting to constant changes in priority.
Applying judgment and discretion in resolving problems and interpreting policies and regulations.
Ability to:
Establish and maintain accurate records of assigned activities and operations.
Interpret and implement local policies and procedures; written instructions, general correspondence; Federal, State, and local regulations.

While performing duties of job, employee is occasionally required to stand; walk; sit; use hands to finger, handle or feel objects, tools or controls; reach with hands and arms; talk and hear.
Employee must occasionally lift and/or move up to 15 pounds.
