1:1, evidence-based physical therapy
in the places our patients live and thrive— private residences, senior living communities, and residential care facilities across South Carolina .
We’re looking for a
licensed Doctor of Physical Therapy (DPT)
who values
patient-centered outcomes, clinical autonomy, and real-world functional rehabilitation .
What You’ll Do
Perform comprehensive
PT evaluations and reassessments
Design and update
individualized, outcome-driven plans of care
Deliver skilled interventions including:
Manual therapy, trigger point release, and myofascial techniques
Balance, gait, and functional mobility training
Therapeutic exercise and neuromuscular re-education
Post-operative orthopedic rehabilitation protocols
Pain science and progressive mobility programs
Provide
caregiver and patient education
for long-term independence
Maintain timely, compliant
EMR documentation
Coordinate care with physicians and interdisciplinary clinical partners
Support safe, structured
discharge planning
Clinically supervise PTAs per state and payer guidelines

Post-surgical recovery
Chronic pain and musculoskeletal dysfunction
Neurological conditions affecting movement
Pediatric through geriatric functional rehabilitation needs
Requirements
Active, unrestricted Physical Therapist license in South Carolina
Accredited
Doctor of Physical Therapy (DPT)
or equivalent degree
Valid
NPI (Type 1)
Current
CPR/BLS certification
Strong clinical judgment and ability to work independently in a mobile model
Excellent communication and caregiver engagement skills
Commitment to compliance and high clinical quality standards
