We are seeking a compassionate Hospice Nurse Practitioner to provide holistic end-of-life care in Greenfield, WI.
Conduct patient assessments and develop individualized care plans
Administer medications and treatments as prescribed
Collaborate with interdisciplinary teams to ensure quality outcomes
Educate patients and families about hospice services
Monitor progress and adjust care plans accordingly
Maintain accurate documentation and participate in discharge planning
Advocate for patient needs within the healthcare system
Qualifications include a Master's in Nursing with hospice focus, current WI Nurse Practitioner license, and preferred hospice/palliative experience. Must have a valid driver’s license, be willing to travel 100%, and possess strong clinical and communication skills.
Benefits include a salary starting at $115,000, 401(k), health/dental/vision insurance, paid time off, and mileage reimbursement.
