We are currently seeking a
Registered Dietitian (RD)
to join our dedicated clinical team at
Richfield in Salem, VA . This is an excellent opportunity for an RD passionate about
clinical nutrition ,
medical nutrition therapy , and
long‑term care .
Key Responsibilities:
Provide
nutrition counseling
and education to residents and families
Complete
MDS assessments
and participate in care planning for high‑risk residents (e.g., feeding tubes, dialysis)
Develop, oversee, and monitor individualized
clinical nutrition care plans
Complete/oversee all
nutrition consults
ordered by healthcare providers
Monitor, document, and report
significant weight changes
Educate and in‑service facility staff regarding
nutrition policies
and best practices
Attend care plan meetings, risk meetings, and interdisciplinary team meetings
Mentor/Precept CDM candidates as assigned
Monitor
quality measures
to identify residents at nutritional risk
Conduct audits related to nutritional programs and kitchen sanitation
Consult with Dining Services leadership regarding menu planning and regulatory compliance
Review nutrition and dining policies in preparation for state surveys
Prepare monthly
Quality Assurance
reports and attend QA meetings
Maintain compliance with all facility safety, health, and regulatory guidelines
Required Qualifications:
Ability to stand, walk, bend, kneel, and lift/move over 50 lbs frequently
Strong communication skills and ability to work with residents, families, and staff
Ability to follow written and verbal instructions
Preferred:
Experience as a Registered Dietitian in
long‑term care
or
skilled nursing facility (SNF)
