Responsibilities
Assessing and monitoring patients' cardiovascular health
Developing and implementing individualized rehab plans
Educating patients on lifestyle modifications and medication adherence
Collaborating with healthcare teams to ensure optimal patient outcomes
Documenting patient progress and adjusting care plans accordingly
