Key Responsibilities:

Coordinate and deliver high-quality hospice care in accordance with physician orders and organizational policies.

Conduct comprehensive assessments of patient health, psychosocial, and spiritual needs.

Develop and update individualized care plans, ensuring ongoing evaluation and adjustment.

Provide direct patient care and supervise ancillary staff, including hospice aides and volunteers.

Facilitate communication with physicians, families, and other healthcare providers to ensure seamless care.

Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.

Support families through bereavement and provide follow-up care as needed.

Maintain accurate documentation of all patient interactions and care activities.

Assist with discharge planning and coordinate resources for continued support.

Required Skills & Qualifications:

Valid RN license in Massachusetts in good standing.

Associate’s degree in nursing required; Bachelor’s degree preferred.

Minimum of three years of nursing experience, including at least one year in an acute care setting.

Strong assessment skills and ability to respond effectively to diverse patient needs.

Knowledge of hospice philosophy, principles, and practices.

Excellent communication and teamwork skills.

Valid driver’s license, insured vehicle, and willingness to travel within the Danvers, MA area.

Ability to work independently, demonstrate flexibility, and handle emotional aspects of hospice care.
