Summary The Community Health Worker (CHW) serves as a trusted liaison between community members and healthcare providers, building strong relationships to improve access to care, increase health literacy, and support overall well‑being. CHWs educate individuals on insurance coverage options, help establish connections with primary care and/or specialty care providers, and guide appropriate use of healthcare services.
