Position Summary

The Nurse Practitioner - PPV will provide care to patients across the Corpus Christi area, supporting those with chronic, complex, or life‑threatening conditions. This role manages palliative and hospice care responsibilities, collaborates with the PCP, specialists, and the interdisciplinary team (IDT), and ensures compliance with practice standards and codes. The Nurse Practitioner will also maintain proficiency in hospice and palliative care procedures and collaborate with the Medical Director.

Responsibilities

Performs routine and emergency assessments for hospice care, including:

Recertification visits for patients entering their 3rd benefit period or beyond.

Emergency assessments as indicated by departmental policy.

Documentation of observations, treatments, and outcomes.

Communication with the Medical Director, attending physician, DCS, and IDT regarding findings and symptom‑relief measures.

Physical and emotional support to patients and families during crises.

Prevents unnecessary hospitalizations by meeting patient and family needs in the home. Additional outpatient responsibilities include:

Bedside palliative care evaluation at the patient’s residence.

Assisting with diagnosis, treatment, and management of acute and chronic conditions.

Ordering, performing, and interpreting laboratory and radiology tests within scope.

Prescribing medications, where delegated and licensed.

Ordering treatments and durable medical equipment as indicated.

Providing other therapeutic or corrective measures as needed.

Consulting with palliative care physicians and informing primary physicians of services provided.

Coordinating palliative care referrals.

Maintaining accurate patient records and charts for reimbursement.

Complying with applicable laws and regulations regarding collaborative agreements.

Initiating reimbursement for services rendered.

Assisting patients and families with goal setting related to treatment options and advanced care planning.

Providing training and continuing education to staff.

Assisting in development of clinical practice guidelines for quality palliative care.

Maintaining confidentiality regarding patients, families, staff, and the organization.

Collaborates with the Medical Director, attending physician, DCS, and IDT to:

Identify patient needs for clinical and psycho‑social interventions.

Communicate those needs to the IDT.

Complete and submit required paperwork within 24 hours of visit completion.

Educates patient and caregiver regarding:

Care of patient.

Disease process.

Dying process.

Symptom control.

Wound care.

Performs other duties as assigned.

Provides face‑to‑face hospice visits, performing routine and emergency assessments as above.

Provides palliative care, performing routine and urgent assessments, and:

Ensures consistent communication with manager.

Maintains at least four days of availability per month.

Triages consults by patient acuity.

Performs bedside education for patients and families.

Orders, performs, and interprets tests as required.

Prescribes medications.

Orders treatments and equipment.

Provides other therapeutic measures.

Consults with palliative care physicians and collaborates with other physicians.

Assists in care coordination and documentation.

Completes accurate billing and coding per CMS guidelines.

Engages in professional growth activities.

Adheres to confidentiality practices.

Collaborates with the Medical Director, attending physician, hospital staff, and IDT for monthly meetings and communication.

Educates patient and caregiver regarding care, disease, goals, symptom control, treatment options, prognosis, and advance care planning.

Supervises nurse trainees and participates in the supervision of residents, fellows, and social‑work or psychology trainees.

Education and Experience

Master’s degree in Nursing required.

For hospice care: Minimum of 1 year nursing practice or equivalent experience; hospice, oncology, home health, and/or long‑term care preferred.

For palliative care: Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred.

Skills

Mathematical: Ability to add, subtract, multiply and divide in all units of measure, including rates, ratios, and percentages.

Language: Ability to read, analyze, interpret business periodicals, technical procedures, and regulations; write reports, correspondence, and manuals; present information and respond to questions; strong written and verbal communication.

Other: Fluency in English; articulation of hospice philosophy; strong organization and time‑management; willingness to work flexible nights, weekends, and travel; maintain confidentiality of information.

Certifications, Licenses, and Registrations

Active and unencumbered CNS or Nurse Practitioner license in the state of employment required.

Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric settings required.

Active NPI number required.

Maintain active DEA certification if state permits prescription of controlled substances by APPs.

Board certification in Hospice and Palliative Care preferred, but not required.

State Specific Requirements

All states except Iowa: Current collaborative physician agreement in place.

Physical Demands and Work Environment The demands of this role require regular standing, walking, and manual dexterity, and the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and other typical healthcare conditions is expected. Routine use of standard medical equipment and tools is essential.
