The Skilled Rehab team at Good Samaritan Algona and Forest City seeks to hire a PRN Occupational Therapist to join our team. The position is PRN, covering the Skilled Nursing Facilities. Currently, we treat the following conditions: orthopedic, weakness, balance/falls/gait, cervical/back pain, general deconditioning, and other various conditions. New grads are welcomed! We are transitioning these locations to an in-house therapy model from contract use. We are looking to hire a candidate who can complement our team. Job Summary Assesses clients/patients/residents and designs occupational therapy treatments. Helps clients/patients/residents develop, maintain and recover their ability to perform tasks in their occupations of life. The therapist conducts evaluations and provides interventions for clients with functional, cognitive or behavioral impairments. Occupational Therapist evaluates physical, cognitive, developmental and psychosocial abilities of clients; develops realistic therapy goals and administers appropriate programs. Documents client information for evaluation; establishes therapeutic plans and modifies them as necessary. Provides education designed to enhance understanding and improve functional outcomes. Demonstrates and incorporates knowledge of current research into daily treatment. May treat individuals of all ages with varying diagnoses and disorders. Demonstrates documentation in accordance with departmental guidelines and meets all regulatory requirements. Exhibits flexibility with variable work schedules and assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Interacts with providers, case managers, family members, third‑party payers, public organizations, etc. May supervise assistants, aides and students as they provide patient care. Expected to provide ongoing education and training to peers as determined by department leadership. Performs other duties as requested by manager. Work may focus on post‑acute, long‑term care and home health care environments.

Qualifications

Appropriate education level in accordance with state licensure.

Must be licensed in the state(s) of practice as an Occupational Therapist.

When working at a Sanford Health Facility (preferred for GSS Facilities): must be certified through the National Board of Certification in Occupational Therapy (NBCOT).

May require a valid driver’s license when applicable.

Must obtain and maintain required department‑specific competencies and certifications.

Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

In South Dakota and Iowa, new graduates under testing with the board will be considered with a temporary license.
