Registered Nurse (RN)
to join our
Home Health & Hospice Department
in
Galion, Ohio .
Position Overview The Home Health and Hospice Registered Nurse – Weekend On-Call is responsible for the overall care coordination and response to needs that arise during the assigned on-call shift. Ensures the timely delivery of appropriate service, which promotes quality and cost‑effective health care outcomes, working within established agency policies and Home Health and Hospice regulations. Works closely with visiting nursing staff, providing direct patient care to ensure that care is goal‑directed and patient needs are being met. The HHH Registered Nurse – Weekend On-Call acts as a patient advocate and primary contact for patients, caregivers, family, insurance companies, doctors, and staff to ensure effective communication regarding patient care.
Qualifications
Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S.) from an approved school of nursing.
Current State of Ohio Registered Nurse licensure.
BLS certification or obtained within 30 days of hire.
Minimum of two years’ previous home health or hospice experience, or in combination with supervisory experience.
On‑Call two (2) weekends per month, 8 a.m. Saturday – 8 a.m. Monday.
Fully remote, unless a visit is needed.
$300/weekend plus per‑visit rate for visits made.
