Assist in developing personalized service care plans for long-term care members
Evaluate member needs and recommend plans for optimal healthcare outcomes
Coordinate services between members, families, caregivers, and care providers
Provide resource support to members for various needs, including housing and employment
Monitor and document care plans to ensure compliance with regulations
What is Required (Qualifications)
Bachelor's degree in a relevant field and 1 year of related experience
RN license in Illinois or equivalent qualifications in human services
Experience working with diverse populations, including racial and ethnic minorities and individuals with specific health needs
Strong understanding of long-term care services and resources
Excellent communication and interpersonal skills
How to Stand Out (Preferred Qualifications)
Experience in case management or a related healthcare role
Familiarity with community resources and support services for long-term care
Knowledge of state and federal healthcare regulations
Additional certifications in relevant fields, such as social work or nursing
Experience with holistic care management approaches
