Nurse Practitioner- Hospice/Palliative/Housecalls
Full-time Nurse Practitioner position focused on hospice, palliative, and house call care in Idaho.
Responsibilities
Collaborate with a multidisciplinary team to provide compassionate, comprehensive care for patients with advanced or terminal conditions, primarily in patients' homes or care facilities.
Conduct face‑to‑face visits for care planning, symptom management, and regulatory compliance, supporting aging in place.
Qualifications
Valid NP license in Idaho and/or Oregon.
Driver's license.
Willingness to obtain additional state license if needed.
Willingness to travel up to 75% of the time.
