At Covenant Community Care a faith‑based, charitable, non‑profit Community Health Center, we provide medical, dental, and counseling health care services to the people of Metro Detroit. As a Federally Qualified Health Center (FQHC), we offer integrated health care services to everyone, regardless of their ability to pay.
Responsibilities
Provide counseling to children and adolescents.
Provide parent education, coaching, and training.
Advocate on behalf of the child and family to assist them in meeting their needs.
Work as part of an interdisciplinary team to recognize, treat, and manage mental health, substance‑use, and psychosocial issues.
Support patients in their health goals, including treatment compliance, sleep, coping with persistent health issues, or others.
Conduct mental‑health and substance‑use screening; provide referrals when clinically appropriate.
Provide psychoeducation to patients and families regarding mental‑health and substance‑use issues.
Conduct assessments, develop treatment plans, and provide evidence‑based therapy in individual, family, or group settings.
Document patients’ progress and diagnostic information in the treatment chart.
Communicate patient progress with the care team as appropriate.
Maintain confidentiality of patient health information in accordance with HIPAA, 42 CFR part 2 and other applicable regulations.
Cover other behavioral health provider’s patients as needed.
Maintain compliance with MI licensing requirements, including completing continuing education.

Perform other duties as assigned.
Qualifications
Licensed Master’s Social Worker (LMSW) or Licensed Professional Counselor (LPC) – required.
Current valid professional licensure in the State of Michigan.
Preferred: English/Spanish language skills.

Maintain professionalism and confidentiality with a sensitive patient population.
Service‑oriented mindset.
Excellent verbal and written communication skills.
Good judgment and independent decision‑making.

Comfortable working with a variety of ages.
Computer competent.
Spanish‑English bilingual language skills are a plus.
