Sentara Home Care is seeking an RN to join our team. City/State: Dumfries, VA. Position Location: Woodbridge, VA. Shift: First (Days). Pay Range: $38.66 - $64.
Position Status: Full Time - Day Shift. We provide market‑competitive compensation packages, inclusive of base pay, incentives, and benefits.
Role Responsibilities The Direct Care Registered Nurse – Home Health and Hospice develops, implements, and evaluates plans of care for specific patient populations. The RN manages a patient case load through coordination and collaboration with multidisciplinary team members to facilitate integrated and comprehensive care, and possesses clinical knowledge and skills to meet standards required by specific clinical areas.
Education
Associate’s Degree (Required)
Certification / Licensure
RN – Active in Virginia (Required)
BLS (Required within 90 days)
Experience
New grads welcome.
Previous Home Care experience (highly preferred).
Experience in developing, implementing, and evaluating plans of care for specific patient populations.
Experience managing a patient case load through coordination and collaboration with multidisciplinary team members.
Clinical knowledge and skills to meet standards required by specific clinical areas.
