They provide outreach and engagement, care coordination, peer support, supportive housing, and rehabilitation to adults with serious mental illness, and promote access to treatment, employment, and other supports and services according to an individualized plan that addresses a client’s strengths, needs, goals, and choices. What You’ll Do
Engage individuals through community-based outreach Provide short‑term counseling and clinical support Support transitions from homelessness to housing Coordinate care across behavioral health, medical, and social services Participate in crisis intervention, discharge planning, and care coordination Collaborate closely with case managers, nurses, and peer staff What We’re Looking For
Master’s degree in social work, counseling, psychology, nursing, or related field Active NYS licensure (LMSW, LCSW, or LMHC) Interest in outreach-focused, field-based work Experience with homelessness or serious mental illness a plus Strong documentation and collaboration skills Details
