Services Coordinator (On‑Site Work Required) Marion County Health and Human Services is seeking an organized, motivated, and person‑centered Services Coordinator to join our I/DD team.

The role provides case‑management services to individuals with Intellectual or Developmental Disabilities (ID/DD) in Home and Community‑Based settings.

General Statement of Duties

Facilitate intake and eligibility determination for applicants.

Conduct independent functional needs assessments.

Plan and carry out case‑management functions.

Supervision

Receives supervision from a Clinical Supervisor who assigns work, establishes goals, and reviews outcomes.

Does not exercise supervisory responsibility.

Typical Duties

Provide services in office and field; drive to community meetings and home visits.

Choice Counseling:

Offer choice annually and as needed of all available service, provider, and setting options.

Service authorization/Individual Support Planning:

Facilitate development of Individual Support Plans for all individuals receiving K‑plan funded services; authorize services and supports per Oregon Administrative Rule 411‑415‑0070; complete enrollment, termination, and transfer forms.

Functional Needs Assessment:

Facilitate or participate in the annual assessment process per current ODDS procedure.

Plans and Annual Summaries:

Complete annual summary for individuals not enrolled in any K‑plan funded service (case‑management only).

Coordination with school:

Attend Individual Educational Plan meetings for students; mandatory participation for individuals aged 15+ to complete transition planning.

Documentation:

Maintain accurate client records and progress notes per Marion County policy.

Service Monitoring:

Monitor services per Oregon Administrative Rule 411‑415‑0090; report serious events per guidelines.

Information and Referral:

Provide timely referral and assistance to individuals regarding available services.

Coordination with other agencies:

Coordinate with community partners to ensure safety and support needs are met.

Staff Meetings:

Attend staff, vacancy, and team meetings.

Other duties as assigned by supervisor.

Response Level 2 In emergencies, employees are expected to report to work promptly, may perform outside‑of‑assigned duties, and may be required to work an extended or flexible schedule. Response‑level staff must complete Incident Command System 100 and 700 training (online or in‑person).

Requirements

Five years of education in a related field or paraprofessional experience/training providing services to individuals with a primary diagnosis of developmental disabilities at a management or case‑management level.

Knowledge of the public service system for developmental disability services in Oregon.

OR any satisfactory combination of work, education, training, or experience relevant to the position as determined by Marion County.

Special Requirements

Must possess a current driver’s license and acceptable driving history; driving record reviewed per county policy.

Finalists must pass a criminal‑history background check.

Position is safety‑sensitive; final candidates must pass a pre‑employment drug screen, including testing for marijuana.

Must not be excluded from federal health‑care programs or federal procurement activities.

Position represented by a union.

Full‑time position eligible for overtime.

Typical work schedule: Monday‑Friday, 8:00 am‑5:00 pm (dependent on departmental needs).

Knowledge, Skills, and Abilities

Professional conduct in accordance with a code of ethics and confidentiality.

Working knowledge of developmental disability concepts and the Oregon public‑service system.

Advanced skill in coordinating and/or providing services to individuals with intellectual or developmental disabilities or similar populations.

Ability to build positive relationships with employees, agencies, and the public; prepare concise reports; document services in a timely manner.

Physical Requirements Must drive, see with depth perception, sit, stand, move within work area, bend forward, climb stairs, lift and carry up to 5 lb. for up to 30 ft., operate keyboard/computer, read 12‑pt font, speak with clear audible voice, hear normal speech level, work in uneven areas; may be exposed to lice, scabies, strong odors during home visits.

Public Service Loan Forgiveness Marion County qualifies for the Public Service Loan Forgiveness Program.

Full‑time employees may qualify for forgiveness of the remaining balance on Direct Student Loans after 120 qualifying monthly payments under a qualifying repayment plan.

Visa Sponsorship Marion County does not offer visa sponsorship.

Within three days of hire, applicants must complete the U.S. Department of Homeland Security I‑9 form confirming authorization to work in the United States.
