Job Summary and Qualifications Your goal as a mental health professional is to create discharge plans that address patients’ psychological, social, cultural, and financial needs, helping them recover and maintain overall well‑being. This role acts as a mediator between clinical teams, patients, families, and other care providers to ensure seamless, comprehensive care.

Responsibilities

Conduct individual, group, and family psychoeducational groups.

Contribute to treatment plan development and attend treatment planning meetings and reviews on an ongoing basis.

Assess environment and patient safety status and report or respond to actual and potential safety issues or changes in the patient’s condition.

Identify risk factors, screening criteria, and interventions for patients with special precautions such as suicide, homicide, falls, or elopement.

Complete relevant assessments, group notes and all other documentation promptly and in compliance with requirements.

Collaborate with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.

Conduct post‑discharge follow‑up as indicated.

Qualifications

Nonviolent Crisis Intervention certification must be obtained within 30 days of employment start date.

Certifications as an Addictive Counselor (AC).
