Sunbelt Staffing is currently seeking a dedicated and experienced Home Health Licensed Practical Nurse (LPN) for a rewarding 13-week contract assignment in Butler, GA. The ideal candidate must have at least 1 year of recent home health experience and possess proven proficiency with HomeCare HomeBase (HCHB) within the last year. A current American Heart Association BLS certification is also required.
Position Details
Contract Duration: 13 weeks
Weekly Hours: 40 hours
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
On-Call: Discussed in an interview
Required Qualifications
Active LPN license in the state of assignment or compact license
Minimum 1 year of recent home health experience
Demonstrated HCHB (HomeCare HomeBase) experience within the last 12 months – this is a strict requirement
American Heart Association BLS certification
Key Responsibilities
Conduct patient assessments and reassessments in the home setting
Collaborate with physicians and interdisciplinary teams to implement individualized care plans
Administer medications and perform skilled nursing procedures (e.g., wound care, IVs)
Document care using HCHB and maintain timely, accurate clinical records
Educate patients and caregivers on health management and treatment plans
Participate in case conferences and contribute to ongoing care coordination
Respond to emergencies and initiate appropriate interventions
Comply with all agency policies, procedures, and applicable regulations
Maintain professional development and stay updated on best practices
