Perform routine supervisory visits to evaluate the plan of care and supervise the delivery of excellent client service. Communicate with Case Managers and other members of the care team REQUIREMENTS Graduation from an accredited school of nursing and current practical nurse licensure. At least two years of practical nursing experience. Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
