Case Manager Pawtucket HH ACT II Team $2500 retention bonus
This position is eligible for a $2500 retention bonus.
Job Summary:
The Providence Center Case Manager Pawtucket HH ACT II Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence.
Duties and Responsibilities
Coordinate a variety of services.
Provide support counseling, problem solving, contracts and limit setting.
Establish attainable goals with the consumer.
Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, including work with substance abusers as it relates to case management.
Present verbally a synopsis of actual cases as required.
Accompany consumers to appointments with agencies, clinics, doctors, etc.
Make assessments and provide appropriate crisis interventions and risk management as needed.
Assess consumers' unmet needs (psychological, substance use, physical, housing, psychiatric).
Identify, assess and respond to crisis situations.
Teach consumers adult daily living skills.
Maintain a clean, healthy and safe environment at all times.
Initiate treatment and advocate for consumer within network of care.
Schedule appointments for consumers within The Center and with outside agencies.
Assist consumers with benefits acquisition (social security supplemental insurance (SSI), food stamps, advocates office, medical doctor).
Complete required interagency and other necessary paperwork.
Identify, obtain, and maintain housing and necessary household goods and furnishings.
Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Attend court and mental health reviews.
Transport consumers in own vehicle or Center van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences.
Other duties may be assigned.
Requirements
Associate's degree required; Bachelor's degree from accredited college or university in Human Services, Social Work, or related field preferred.
Experience in Human Services Field preferred.
Must have a valid driver's license, automotive insurance, and registration.
Ability to read and interpret documents such as policy and procedure manuals.
Ability to write routine reports and correspondence.
Ability to speak effectively before team members and other employees and have the capacity to work with other community agencies.
