Upon orders of the physician, Social Worker identifies factors which influence a patient’s and/or family’s ability to respond to a medical therapy program. Through consultation and home visits, develops means to meet the goals of the plan of care. The Medical Social Worker is responsible for the coordination, delivery and documentation of Medical Social Services and to report to the appropriate Team.
Job Duties / Responsibilities
Assess patient’s social, emotional and safety factors related to his/her illness, taking into consideration the environment and present support system.
Evaluate patient/caregiver decision‑making ability.
Assist patients/caregivers in recognizing deficiencies in meeting their care needs.
Allow and encourage patient/caregivers to participate in identifying care needs if patient/caregiver is capable.
Identify needs and establish a plan of care.
Conduct timely assessment as specified by Nurse Coordinator/Manager or per Agency protocol.
Implement an action plan based on assessment of needs.
Refer alleged patient abuse and/or neglect concerns or unsafe situations to proper authorities timely.
Maintain current knowledge of resources available; mobilize available resources within existing time constraints.
Provide feedback to referral resources timely.
Involve extended family, with permission of patient/primary caregiver, to assist in problem resolution.
Complete documentation timely and accurately.
Complete forms/electronic communications according to Agency policy procedure within established time frames.
Document follow‑up communications with patients/families.
Document assessments, plans and resources contacts per established documentation protocols.
Respond to and address documentation deficiencies within two (2) days of receiving notification of deficiency.
Coordinate care with inter‑disciplinary team.
Report patient problems or changes in situation to Nurse Coordinator, Nurse Manager, physician and/or team members timely and appropriately.
Attend, participate and document case conferences as scheduled.
Communicate with patients, significant others and interdisciplinary team members in a positive, respectful and caring manner.
Participate in Quality Improvement activities.
Perform other duties as assigned.
Requirements Formal education required:
Master’s Degree in Social Work.
Licensing:
Licensed to practice in New York State as a Medical Social Worker (LMSW or LCSW required).
Previous Experience Required:
Two years of appropriate experience as a Medical Social Worker.
Summary Community/home care experience is preferred.
