Responsible for coordinating/assuring a high standard of hospice patient care consistent with each profession on the interdisciplinary team. Supports the Hospice House Manager in assuring continuous, quality, cost‑effective operations and promotes positive teamwork focused on delivering a high standard of care. Essential Functions

Adhere to all policies, procedures, and regulations regarding patient care, safety, fire, security, and risk management. Provide a safe environment for patients, guests, visitors, and employees. Provide leadership, problem‑solving, and decision‑making in management. Facilitate communications and work effectively between facility staff, patients/families/caregivers, physicians, vendors, and other departments and professionals as appropriate. Serve as liaison with other departments to resolve problems and improve efficiency and effectiveness. Schedule RN/LPN/CNA staffing in collaboration with Hospice House Manager per ORS 441.155 and OAR 333, Division 510 regulations. Approve/schedule RN/LPN/CNA time‑off requests and arrange alternate staff coverage in response to patient acuity and unscheduled staff absences. Review timesheets and communicate staffing and scheduling concerns with the Hospice House Manager. Actively participate in staff meetings, department meetings, management meetings, in‑services, competency training, and other related activities. Evaluate the quality of care delivered by professional staff, as evidenced by clinical assessments and medical record review. Monitor documentation and implementation of physical, psychosocial, and spiritual interventions for the patient/family/caregiver. Provide leadership and formal/informal education to staff through in‑services, case conferences, and group discussions on hospice philosophy, operations, policy, and procedures, and problem‑solving. Articulate the needs of patients transferring between levels of care to staff and promote continuity of care. Provide direct patient care up to 25% of working hours. Ensure IDT notes and orders are signed in a timely manner. Assist with employee hiring decisions and serve as orientation preceptor; provide ongoing education to patient care staff. Supervise facility staff as assigned; complete timely employee evaluations. Rotate after‑hours calls for Hospice House and participate in various duties related to patient care. Actively participate in quality assurance, utilization review, and risk management activities. Facilitate patient care and quality management activities by assuring staff participation and facility representation on focus groups; follow up on surveys and grievances; perform medical record and utilization review audits. Required Skills & Qualifications

Graduate of an accredited school of nursing with unencumbered licensure to practice professional nursing in the State of Oregon. 2 years of supervisory experience preferred. Excellent assessment, problem‑solving skills, and nursing skills per the competency checklist. Strong clinical judgment and patient triaging skills, efficient time management with minimal oversight, and communication skills with other care team members and physicians. One to two years of recent medical/surgical care experience in an institutional setting. Ability to pass a background test, drug screen, and TB test. Current Food Handler’s certification required. Valid CPR Certification. Strong communication and interpersonal skills. Ability to work independently and collaboratively within an interdisciplinary team. Compassion, empathy, and dedication to patient care. May be exposed to infections and contagious diseases under a wide variety of circumstances. Handles emergency or crisis situations. Physical demands include the ability to lift patients, assist with ambulation, walking, carrying, and pushing.
