Position Summary The Case Manager is responsible for coordinating individualized services and providing support that promotes client stabilization, retention, and successful transition from detox and residential care to aftercare. This role integrates clinical, vocational, and social services into treatment planning, ensuring clients have the resources and structure needed for recovery. Case Managers work closely with therapists, utilization review, operations, and outside providers to ensure a seamless continuum of care that meets DHCS, Joint Commission, company and insurance standards.
Key Responsibilities
Conduct client introductions and assessments within 48 hours of admission
Manage a caseload of 12–16 clients with weekly check-ins
Assist with FMLA, disability paperwork, and outside appointments
Facilitate 1–3 groups per week
Aftercare & Transition Planning
Initiate aftercare planning at admission
Develop relapse-prevention plans and coordinate discharge to OP/IOP/PHP
Connect clients to sponsors, peer networks, and community supports
Communicate with families and ensure safe transitions
Documentation & Compliance
Complete all documentation in KIPU by end of shift
Track discharge dates and support utilization review
Maintain records that meet DHCS, Joint Commission, and insurance standards
Collaboration & Communication
Liaise with probation officers, attorneys, and outside agencies
Participate in treatment team and family meetings
Provide updates to referral sources and weekend operations
Qualifications
Registered/certified SUD counselor (CCAPP, CADTP, CAADE) or BBS‑registered/licensed (AMFT, ACSW, APCC, LMFT, LCSW, LPCC)
Minimum 6 months’ experience in case management or treatment/social services (detox/residential preferred)
Current TB test, First Aid, and CPR certification
If in recovery, minimum 2 years continuous sobriety
Skills & Competencies
Strong communication and rapport-building
Knowledge of housing, employment, legal, and social resources
Familiarity with DHCS, Joint Commission, and insurance standards
Counseling skills: motivational interviewing, relapse prevention, de‑escalation, and co‑occurring disorder support
Physical Demands & Environment
Ability to sit, stand, walk, and lift up to 20 lbs.
Work in detox/residential settings with clients in emotional or behavioral crisis
Exposure to indoor/outdoor conditions; reasonable accommodations available
