What You'll Do
Provide in-home OT services under the direction of a licensed Occupational Therapist
Follow and carry out established plans of care
Educate patients and caregivers on safety, ADLs, and adaptive techniques
Monitor patient progress and report changes to the supervising OT or nursing team
Complete accurate, timely documentation in accordance with agency guidelines
Participate in interdisciplinary collaboration as needed
Promote patient safety, infection control, and use of proper equipment
Maintain confidentiality and compliance with agency, state, federal, and ACHC standards
What We're Looking For
Licensed Occupational Therapy Assistant (OTA/COTA)
Home Health experience preferred, but not required
Strong communication and documentation skills
ability to work independently in a home-based setting
professional, dependable, and patient-focused approach
Physical ability to perform in-home visits
