Job Description Summary
Provide medical social services to patients in their homes, in accordance with physician orders and under the direction of the Director of Nursing or other supervisor.
How Will You Make An Impact & Requirements As a Medical Social Worker you will assess clients’ coping abilities, consult with the health team, facilitate interdisciplinary care, and help families access community resources.
Responsibilities
Assess client’s potential to cope with social and health problems.
Consult with health team members to explain social, emotional, and environmental factors related to client health.
Help clients utilize family and community resources.
Assist with client admission to the Agency by performing initial psychosocial evaluation and home assessment.
Co‑develop and implement interdisciplinary care plans for medical social work.
Observe, record, and report changes in emotional and social factors that affect client care and treatment response.
Consult with the attending physician regarding plan adjustments.
Enter clinical notes and progress notes into the EMR within required time frames (24 hrs. for non‑Oasis visits / 48 hrs. for Oasis visits).
Evaluate client and family responses to the medical social work intervention for effectiveness.
Confirm weekly scheduling of visits with the Director of Nursing to coordinate other staff visits.
Participate in agency activities, committees, staff development, and in‑service education as appropriate.
Supervise the social work assistant monthly.
Participate in discharge planning.
Continuously pursue professional development through formal education, workshops, conferences, and research.
Maintain compliance with the agency dress code policy.
Job Conditions
Must communicate effectively both orally and in writing.
Occasionally required to bend, stoop, reach, and move client weight up to 250 lbs and lift or carry up to 30 lbs.
Requires adequate hearing, vision, and physical dexterity to perform assessments and clinical care.
Must be able to assess client homes that may not be wheelchair accessible.
Maintain current TB testing in accordance with the Health Screening Policy.
Report to work at assigned or scheduled times and follow procedures for absence or time‑off notifications.
Qualifications
Master’s Degree from a School of Social Work approved by the Council of Social Work Education.
Two years’ experience, with at least one year in a healthcare setting (hospital, clinic, rehabilitation center, etc.).
