Part-Time Registered Nurse (RN)
to support our clients and caregivers in a home care setting. The ideal candidate is professional, detail-oriented, and experienced with
WellSky
for documentation and care management. Plenty of room for growth and advancement on the administrative side as well.
Responsibilities:
Conduct in-home assessments, reassessments, and supervisory visits
Develop and update individualized care plans
Document all clinical notes accurately in
WellSky
Provide training, guidance, and support to caregivers
Ensure compliance with agency policies, state regulations, and best practices
Communicate effectively with clients, families, and office staff
Monitor changes in client conditions and adjust care plans as needed
Requirements:
Current and active RN license in the state of Virginia
Experience in home care or community health strongly preferred
Proficiency with WellSky (or similar EMR systems) required
Strong clinical assessment skills
Reliable transportation for client visits
Ability to work independently and manage time effectively
Compassionate, professional, and committed to quality care
