Pre-Transplant Coordinator – Houston Methodist
Pre-Transplant Coordinator is a registered nurse responsible for participating in department initiatives, facilitating, and coordinating all aspects of the transplant referral, evaluation, waiting period, and peri-operative phases of transplantation. The Coordinator demonstrates continuous skill progression, works with an interprofessional transplant team to ensure coordination of care, manages emergencies, and advocates for patients and families.

Qualifications Education Bachelor’s degree or higher from an accredited School of Nursing
Experience Four years nursing experience or three years’ Oncology/BMT-specific experience for area of specialty Previous transplant acute care and/or ICU experience preferred
Licenses and Certifications RN – Registered Nurse – Texas State Licensure – Texas Board of Nursing_PSV Compact Licensure – Must obtain permanent Texas license within 60 days (if establishing Texas residency) BLS – Basic Life Support or Instructor (AHA) – American Heart Association
Skills and Abilities Demonstrates skills and competencies necessary to safely perform the assigned job, determined through ongoing skills, competency assessments, and performance evaluations Sufficient proficiency in speaking, reading, and writing English Effective communication with patients, physicians, family members, and co-workers Strong assessment, organizational, and problem-solving skills Ability to manage complex patient caseload and prioritize/triage based on patient status Professional image presentation Knowledgeable of OPTN/UNOS/NMDP/CIBMTR/FACT policies and CMS Conditions of Participation Participates in department donor awareness events, educational opportunities, and community activities Assists physicians in perioperative setting Working knowledge of Microsoft products
Essential Functions People Essential Functions Uses therapeutic communication in an active, positive, and effective manner with all transplant team members, patients, and referring physicians, reporting pertinent patient care information. Identifies and responds to clinical, psychosocial, and economic needs of transplant patients and families during evaluation, waiting, and pre-operative phases. Fosters teamwork, anticipates needs of team members, proactively offers assistance, and contributes to employee engagement.
Service Essential Functions Coordinates evaluation process for potential transplant candidates, including medical/surgical, psychosocial, educational, laboratory, radiological, and nutritional evaluations. Establishes mutual educational goals with patients and family/caregivers about living donor options and transplant regulations. Recognizes potential problems, abnormal diagnostic findings, and coordinates required care during waiting phase. Collects and evaluates medical, psychosocial, and financial data for review by transplant team. Ensures patients and referees are informed of transplant listing status and provides education at stage. Participates in transplant events and meets departmental and organ program targets for process and patient satisfaction.
Quality/Safety Essential Functions Identifies appropriate candidate upon referral and ascertains evaluation requirements. Maintains comprehensive documentation of evaluation process and candidate progress. Provides accurate data to regulatory agencies and ensures compliance with government reporting requirements.
Finance Essential Functions Validates patient’s insurance authorization for diagnostic testing and documents clinical activities for charge capture. Uses time efficiently during workloads and supports team members.
Growth/Innovation Essential Functions Offers innovative solutions through participation in performance improvement projects. Participates in continuous learning and applies evidence-based practice. Identifies learning needs, seeks educational opportunities, and updates My Development Plan.
