Job Description Family Medicine APP (Family Medicine)
Full-time, Utah Valley Clinic – Provo, UT.

Compensation
Competitive Compensation.
Incentives: Up to $7,500 relocation bonus (if applicable).

Minimum Qualifications
Women's services experience preferred.
Bilingual (Spanish) preferred, but not required.
Current state license as a Physician Assistant or Advanced Practice Registered Nurse (APRN).
Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
Eligible for State Controlled Substance License II through V.
Eligible for Federal DEA Certificate II through V.
BLS Certification for Healthcare Providers.
Other certification as required (e.g. ACLS, PALS, TNCC, NRP).
Preferred Qualifications
Master’s degree. Education must be obtained from an accredited institution. Degree will be verified.

Hourly rate: $56.76 - $87.59 (actual hourly rate dependent upon experience).
