Job Summary Provide physical therapy and related services for patients while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of physical therapy and within organizational guidelines. Provide quality patient care, including screening and evaluation of patients, development of initial and ongoing treatment plans of care, discharge planning, supervision of assistants and rehab techs, and case management responsibilities. Maintain professional conduct that represents Midtown Center for Health and Rehabilitation's commitment to quality and service in all interactions with clients and facilities. Adhere to all applicable state and federal regulations governing physical therapy practice.
Job Responsibilities
Optimize patient functionality by providing, directing, and/or overseeing physical therapy services, including screening, evaluation, and treatment as referred by an approved referral source.
Comply with state and federal guidelines related to accurate and timely documentation and billing within the medical record.
Establish and maintain effective, patient‑centered interactions with patients, interdisciplinary team members, and facility staff.
Refer patients for treatment to Physical Therapist Assistants (PTAs) and provide consultation and clinical supervision.
Directly supervise PTAs and Rehab Techs in accordance with State Practice Acts, OBRA requirements, and policies.
Participate in utilization review, care conferences, case management, administrative meetings, and interdisciplinary team activities.
Develop and enhance physical therapy programs, techniques, and services to support quality outcomes.
Ensure compliance with environmental safety, infection control, emergency preparedness, and patient rights policies.
Enhance clinical expertise through continuing education and professional development.
Maintain valid state licensure and remain informed of developments within the profession and healthcare environment.
Understand and apply relevant payment models and treatment guidelines to ensure clinically appropriate service delivery.
Maintain the confidentiality of the facility and patient information.
Conduct oneself in an ethical, legal, and professional manner at all times.
Perform other duties and responsibilities as assigned.

Ability to perform duties in a healthcare environment with moderate noise levels.

Supervise Physical Therapist Assistants (PTAs), Rehab Techs, and/or Physical Therapy students in accordance with state practice act guidelines, providing consultation and clinical supervision and supporting clinical development in collaboration with the Director of Rehabilitation.
Qualifications
Graduate of an APTA‑accredited school of physical therapy.
Valid state Physical Therapist license.
Proficiency with Microsoft Office applications and basic internet navigation.
