Job Title
Program Coordinator - Shared Care Department
Essential Duties and Responsibilities
Responsible for tracking, coordination and communication of patient referrals
Ensure complete and accurate registration, including patient demographics and current insurance information is accompanied by the referral
Ensures referrals are addressed within a timely manner
Schedule new patients for appointments
Communicate with other physician offices to ensure referrals requirements are met
Compiling medical records, as needed
Answers phones to address questions from patients and staff
Management of compliant program documentation
Assist external Shared Care field Physician Liaisons as needed
Maintains strict confidentiality; adheres to HIPAA guidelines and regulations
Performs other work-related duties as assigned
Qualifications
High School Diploma
2 years’ experience with referral coordination and insurance verification
Knowledge of business office procedures
Ability to foster positive relationships with Shared Care Team and referring partners
Ability to operate a computer, computer software and basic office equipment
