Good Shepherd Hospice
Overview
Good Shepherd Hospice is seeking a compassionate Hospice Intake Coordinator to serve in our Dallas/DFW and surrounding communities. The Hospice Intake Coordinator assists in gathering required patient information and coordinating enrollment of new patients into the hospice program.
Requirements
Hospice Admissions/Intake experience required
Homecare Homebase software experience highly preferred
Responsibilities
Initial contact person for all new inquiries and referrals
Respond promptly to any referral or inquiry
Coordinate the scheduling of intake personnel to follow up with referrals
Provide guidance to hospice staff during the referral and enrollment process
Initiate communication with the RN Case Manager regarding eligibility and enrollment into the hospice program
Inform the RN Case Manager of unusual or potentially problematic patient/family issues
Coordinate all admission enrollment paperwork
Obtain history and physicals, progress notes, lab results, nurse notes, medication sheets, etc., as needed to complete the initial patient assessment
Maintain regular communication with the referral source
Review completed enrollment packet for accuracy and forward for billing and filing
Input initial referral information into the computer
Communicate with medical professionals, the hospice team, and others
Coordinate completion of admission packets
Maintain accurate, up‑to‑date records
Demonstrate strong communication and customer service skills
Qualifications
Hospice Intake experience required
Valid driver’s license
Demonstrated commitment to the hospice philosophy of care
Successfully pass background checks and drug screen
