Overview

Evaluate potential admissions to the facility by reviewing clinical documentation and conducting thorough assessments.

Serve as a liaison to hospitals, conducting field visits and consultations to identify suitable applicants.

Facilitate the application process by gathering psychosocial and financial information from applicants.

Document clinical and psychosocial assessments, ensuring accuracy and completeness.

Coordinate with Managed Care Companies to obtain necessary pre-authorizations for admissions.

Conduct facility tours for potential patients, families, and representatives.

Provide weekend and holiday admission coverage as required.

Collaborate with HMO Case Managers to review skilled needs and optimize admission levels.

Prepare and submit monthly statistics and reports to the Director of Admissions.

Key Responsibilities & Duties

Review PRI’s and supporting clinical documentation for admission appropriateness.

Conduct pre-admission assessments, gathering medical and social information.

Initiate application processes during field visits and telephone consultations.

Document applicant assessments and maintain accurate records.

Coordinate with Managed Care Companies for pre-authorizations and admission approvals.

Provide facility tours to potential patients and their families.

Ensure weekend and holiday admission coverage as needed.

Collaborate with HMO Case Managers to optimize admission levels.

Submit required reports and statistics to the Director of Admissions.

Job Requirements

Bachelor of Science in Nursing (BSN) required; RN registered in the State of New York.

Minimum of 2 years of experience in post-acute admissions processes.

Acute care experience preferred; MDS experience and PRI certification are advantageous.

Proficiency in Electronic Medical Records, Microsoft Outlook, Word, and Excel.

Strong clinical assessment and documentation skills.

Ability to multitask and work accurately in a fast-paced environment.

Knowledge of medical terminology and treatments relevant to adult and geriatric populations.

Bilingual proficiency (English/Spanish) is a plus.

Excellent interpersonal and communication skills for interacting with patients and families.
