Nurse Case Manager I

Base Pay Rangeósitos $68,880.00/yr - $108,240.00/yr
Hours 9 am–5:30 pm EST (Monday–Friday) with 2–3 evening shifts per month: 11:30 am–8 pm EST.
Location Fully virtual full‑time; required in‑person training sessions. Candidatesაწყ must be within reasonable commuting distance. Location: Illinois, New York.
Responsibilities
Ensures member access to services appropriate to their health needs.
Conducts assessments tocarry identify individual needs and develops a care management plan.
Implements the care plan, facilitating authorizations/referrals as appropriate.
Coordinates internal and external resources to meet identified needs.
Monitors and evaluates the effectiveness of the care management plan and modifies as necessary.
Interfaces with Medical Directors and Physician Advisors on development of treatment plans.
Negotiates reimbursement rates as applicable.
Assists in problem‑solving with providers, claims, or service issues.
Assists with development of utilization/care‑management policies and procedures.
Minimum Requirements
BA/BS in a health‑related field and at least 3 years of clinical experience (or equivalent education/experience). Required: current, unrestricted RN license in applicable state(s). Multi‑state licensure required if serving multiple states.
Preferred Skills
Certification as a Case Manager preferred.
BS in a health orત human services related field preferred.
Salary and Benefits
Salary range: $68,880–$108,240 (based on location, experience, etc.).
