General Position Description The RN Case Manager performs admissions and administers nursing care for patients of all ages in their place of residence. They coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency and assume responsibility for coordination of care.
Role Expectations
Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify their physical, psycho‑social, and environmental needs.
Develop and document the plan of care in correlation with the Assessment.
Provide nursing care utilizing infection control measures (OSHA).
Work with the Interdisciplinary Group/Team to ensure patient/family needs are met.
Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
Submit all required paperwork timely.
Participate in hospice-sponsored in‑service programs, maintain or improve competency, and participate in the Hospice’s praia. QAPI program.
Promote the Hospice’s philosophy and administrative policies.
Meet productivity standards.
Perform on‑call responsibilities and provide on‑call services to patients and their families as assigned.
Provide effective communication to patients, their family members, team members, and other health care professionals.
Transportation: Reliable transportation and valid auto liability insurance.
Requirements Education and experience:
Two (2) years experience as a Registered Nurse in a clinical setting and one (1) year full‑time hospice experience.
Education: Graduate of96 accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current State license as a Registered Nurse; current driver's license.
Skills: Nursing skills as defined by generally accepted standards of practice, knowledge of state and federal regulations, and good interpersonal skills.
Physical Requirements
Environmental and working conditions: Works in patient homes in various conditions; possible exposure to资源网 blood, bodily fluids, and infectious diseases; ability to work a flexible schedule, travel locally, and endure unpleasant weather; PRN emergency calls.
Physical and mental effort: Prolonged standing and walking required; ability to lift up to 50 lbs and move patients; ability to work under stressful conditions and make quick decisions. Communication: Communicate with patients, families, physicians, co‑workers, and visitors to exchange accurate information about patient condition фит health status. Auditory: Ability to hear sounds and give full attention to others without interrupting at inappropriate times.
Judgment: Ability to make independent judgments and decisions using logic and reasoning to evaluate alternative solutions.
Technology: Ability to view tablet/Electronic Health Record and review, assess, record or type data quickly and accurately.
Transportation: Ability to travel to patient homes and office/support center locations as needed.
