As a Valued Member Of Our Team, You Will

Conduct comprehensive health assessments, including detailed medical histories, physical examinations, and ordering/interpreting diagnostic tests.

Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.

Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements.

Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications.

Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma).

Perform minor office procedures as trained and within scope of practice.

Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated care.

Document all patient encounters accurately and thoroughly in the electronic health record.

Participate in quality improvement initiatives and adhere to all organizational policies and procedures.

Serve as a resource for medical staff and participate in educational activities as needed.

Provide age‑appropriate care to patients across the lifespan, from infants to geriatrics.

Manage referrals to specialists and coordinate follow‑up care.

Maintain current knowledge of family medicine best practices and engage in ongoing professional development.

Qualifications

1–2 years clinical experience in a primary care/internal medicine setting

Active PA or NP in family medicine specialty certification

Active, unrestricted CT medical license or in process of obtaining

ACLS/BLS/PALS (as required) certification

DEA license

Schedule 40 scheduled hours, Monday‑Friday, 7:30 am‑4:30 pm, with a 4‑day work week available.

Skill Set 1–2 years of clinical working experience as an NP/PA within the given specialty/service line recommended. APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.
