Primary Care Mental Health Integrated (PCMHI) Licensed Clinical Social Worker Valor Healthcare seeks a Licensed Clinical Social Worker (LCSW) or equivalent counselor/therapist (LPC or LMFT) to join the Veteran Affairs Community Based Outpatient Clinic in Harriman/Roane County, TN. The role involves primary care mental health integration (PCMHI) with an all‑adult veteran patient population.

Position Summary PCMHI is an evidence‑based model that expands mental health access, assessment, and treatment in primary care. Clinicians receive training and certification via VA. Sessions last 30 minutes or less, with no more than six per issue per year.

Schedule Part‑time: 16 hours per week; Monday‑Friday, 8:00‑4:30 PM (with flexibility within these parameters).

Core Responsibilities

Performs initial and follow‑up evaluations, including assessments.

Engages the veteran, core, and expanded PACT team members in treatment planning.

Provides a range of treatment services, including individual therapy.

Provides behavior activation Circ short‑term interventions for physical and behavioral health issues such as diabetes, heart conditions, depression, anxiety, and adjustment disorders.

Provides evidence‑based psychotherapies as necessary.

Provides emergency care for all patients enrolled in the CBOC, including referrals to emergency primary care, psychiatric medication evaluation, or coordination of inpatient psychiatric admission.

Offers crisis intervention services, coordinates family conferences, and serves as liaison to family members.

Provides case management to veterans and families throughout the continuum of care.

Addresses issues such as advance directives and organ donation in accordance with acceptable standards.

Inputs all patient information into VISTA/CPRS.

Completes all clinical reminders due at each patient visit.

Participates in program planning and development, making recommendations for policy changes.

Collaborates in performance improvement processes and complies with performance measures required by Valor and VA.

Obtains veteran feedback and works to enhance services.

Complies with all Valor and VA training requirements.

Ensures compliance with OIG, JC, EOC oversight, lab compliance, and other related requirements.

Focuses on clinical outcomes per VA guidelines.

Provides excellent customer service to veterans, families, colleagues, and visitors.

Collaborates with clinic leadership to resolve veteran concerns or complaints.

Participates in enrollment growth initiatives and outreach events.

Supports patient enrollment and retention through appointments and follow‑up visits.

Embraces and supports new initiatives, whether clinical or operational.

Qualifications

Doctoral degree in psychology from an APA, PCSAS, or CPA accredited program; or \-H2 corde ** a. Master’s degree in social work from an accredited program; equivalent degrees (e.g., counseling, marriage and family therapy) considered subject to VA approval. Valid, unrestricted LCSW or equivalent license in the state of employment; able to practice independently at the master’s level.

Minimum of 1‑3 years post‑master direct counseling experience with adult patients.

Credentialed and in good standing through the Veterans Health Administration (VA).

Current certification in Basic Life Support, renewed as required.

Strong computer and EMR skills.
