Case Manager (Work Release)
Location:
Dover, DE and Swedesboro, NJ
Job Overview:
Provide intake, assessment and case management services to Kintock clients, which includes ongoing assessment for program eligibility, monitoring all aspects of care and client services, and ensuring clients’ needs are met. Individual counseling and psycho‑educational classes may be included at the discretion of the Program Director.
Responsibilities
Develop a comprehensive plan of care that meets the client’s needs
Provide the client with information on community and agency services and make appropriate referrals
Educate the client on all aspects of program participation, including rights, responsibilities, and appeal rights
Utilize case‑management skills such as brokering, advocacy, monitoring, and discharge planning to link clients and families to resources and services
Work closely with the client to develop and revise client‑focused care plans that meet client needs
Schedule and/or participate in case conferences with clients/caregivers, plan services, and negotiate agreements
Provide discharge planning for dis‑enrolled clients to ensure their needs are met
Maintain the client’s physical file with required forms as per policy
Ensure all services are supported by a care plan and case‑note documentation
Ensure that all court stipulations are met for each resident assigned to the caseload
Facilitate required Cognitive Behavioral Workshops and classes
Perform all assigned ACA‑related duties
Attend ACA and other mandatory training classes in accordance with Kintock training policies
Provide transportation for residents as needed
Perform other work‑related duties as assigned by the Senior Case Manager or Director
Qualifications / Requirements
Excellent oral and written communication skills
Minimum six‑months of relevant experience preferred
Valid driver’s license
Bi‑lingual in English/Spanish preferred
Knowledge Microsoft Office Suite (Word, Excel, Outlook)
ACA Requirements Required to perform assigned ACA‑related duties (details provided in training).
Abilities and Goals
Ability to follow course curriculum, facilitate groups and/or workshops
Learn, analyze, and interpret contract rules, regulations, policies, and procedures
Participate as an effective member of a program team
Education Requirements High School Diploma (or equivalent) required; Bachelor’s degree in Criminal Justice or a Social Science required.
