Position Summary The Clinical Liaison works collaboratively with business development, admissions, and clinical teams — as well as external referral partners — to facilitate appropriate patient referrals and admissions. This role is responsible for completing pre-admission assessments, reviewing medical records, interviewing patients and families, and determining clinical appropriateness for inpatient rehabilitation admission. The Clinical Liaison ensures CMS/IRF compliance, recommends level of care, and communicates admission status in a timely and professional manner. In addition, this position represents the hospital in community activities, educational events, and outreach opportunities.
Key Responsibilities
Completes thorough and accurate pre-admission assessments to ensure rehabilitation criteria are met.
Maintains timely communication with referral sources, targeting referral-to-acceptance turnaround within two (2) hours.
Follows up on internal and external issues related to referrals and admissions.
Schedules and conducts hospital tours for patients, families, and referral partners.
Identifies new referral opportunities and remains informed about marketplace trends.
Utilizes email and Teams to document and communicate pending patient needs.
Collaborates closely with DBD, CEO, Liaisons, and Admissions to support teamwork and patient flow.
Achieves established performance goals and maintains accurate logs of pending, admitted, and denied cases.
Provides regular updates regarding referral activity, competitors, and market trends.
Understands PPS reimbursement systems, DRGs, and case weights.
Assists team members and supports business development initiatives as needed.
Minimum Qualifications
Current RN, LVN, PT, OT, SLP, or LSW license (or equivalent clinical licensure, as applicable).
Experience in acute rehabilitation, case management, or clinical liaison work preferred.
Strong communication, relationship-building, and organizational skills.
Knowledge of IRF criteria and CMS guidelines strongly preferred.
