Job Summary NP/PA – Community Health Center at Erlanger.

Full‑time day shift (Monday–Friday, 8:00 a.m.–5:00 p.m.).

Provide high‑quality primary care and support services to individuals and families in the local community, working collaboratively with physicians, nurses and other health‑care professionals to deliver patient‑centered care in a community health setting.

Responsibilities

Conduct comprehensive health assessments, diagnose and manage viewed acute and chronic conditions, and develop individualized treatment plans.

Provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention nayo.

Manage follow‑up care, coordinate referrals Condo ineffective when necessary, and ensure continuity of care for patients.

Maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives.

Communicate effectively with patients, educate them about their health, and collaborate with interdisciplinary teams to foster trust within the community.

Address social determinants of health by connecting patients to appropriate resources and support services.

Support team‑based approach to care, participate in practice population management, quality improvement process, and performance evaluation.

Education & Credentials

RN with a master’s degree in nursing and certification as a Family and/or Adult Nurse Practitioner; graduate of a nationally accredited school of nursing.

Experience

Prior experience in a Federally Qualified Health Center (FQHC) strongly preferred.

Licensing & Certification

Current licensure to practice nursing in the State of Tennessee and eligibility to sit for specialty certification.

The Tennessee nursing license must be obtained within three months of hire for non‑Tennessee residents from a compact state.

Preferred: Certification in specialty area(s).

Department Position Summary A nurse practitioner in Family Practice/Internal Medicine works under the direction of physicians/medical director, develops evidence‑based protocols, collects, documents, and analyzes patient data, and collaborates with the health‑care team on testing and treatment.

The role includes community and corporate education on internal medicine topics and the development of screening programs for high‑risk individuals.

Standard Hours Regular full‑time day shift (Monday–Friday, 8:00 a.m.–5:00 p.m.).
