Caregiver / Personal Attendant / Direct Support Professional (DSP)

What you’ll do
Provide personal care assistance and companionship to your assigned client.
Promote your client’s quality of life through meaningful activity and/or conversation.
Provide light housekeeping and meal prep; transportation as appropriate.
Follow care plans/instructions; communicate all changes and concerns with’avez Sage Home Care Manager.
Complete an electronic daily shift report.
Comply with Sage Eldercare’s Policies and Procedures and all applicable laws and regulations.
Who you are
Must have at least one year of experience working with a Home Care agency.
Must have experience working with dementia clients.
Must maintain a valid driver’s license and meet the organization’s auto‑insurance requirements. Occ rotten transport of client(s) is required.
Must be passionate about working with seniors to improve their quality of life.

Compensation
Bronze Pay Rate – 4‑6 hours: $34.50/hr
Silver Pay Rate – 6‑11 hours: $26.72/hr
Overtime (12+ hours): $40.08/hr
Overtime (12+ hours): $22.97/hr (alternative rate for 12‑hour shift)
