Job Summary
This position is within Housing and Homeless services with the Permanent Supported Housing Healthcare Collaborative (PSH HCC). This program is a collaboration between Integral Care, CommUnity Care, Central Health, and ECHO, designed to provide integrated, mobile healthcare to residents in designated Permanent Supported Housing single site projects. It works to fill service gaps for adult residents exiting chronic homelessness with disabling conditions. Clients are highly vulnerable and have complex needs. Preference is given to candidates with a working knowledge of best practices for Permanent Supported Housing. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Responsibilities
Assist clients affected by mental, physical health, and/or substance use concerns in achieving their desired life outcomes.
Provide services in a person-centered, trauma-informed, culturally and linguistically appropriate manner.
Act as a model of recovery by sharing a lived experience, bringing hope, and engaging clients through education, advocacy, and outreach.
Provide individual or group services such as training in recovery strategies, self‑help, self‑advocacy, accessing healthcare services, community resources, or other supervisor‑approved topics.
Collaborate with a treatment team to support each client’s whole health and recovery goals.
Perform job duties in the community and/or clinic.
Qualifications Education:
High School Diploma or GED equivalent required.
License:
Current and valid driver’s license with a clean driving record required. Peer Support Specialist Certificate preferred.
Other Requirements:
Regular, predictable attendance; willingness to work a flexible schedule; ability to use a personal automobile and cell phone.
Hours & Salary Full‑time; Monday – Friday, 8:00 AM – 5:00 PM. Salary: $21.66 per hour.
