Responsibilities

Evaluate occupational therapy patients to determine rehabilitation needs, potential for achievement, and develop a plan of care.

Participate in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.

Make initial therapy evaluation visits and re‑evaluate patients’ therapy needs during each visit. Perform follow‑up assessments according to OASIS and PPS regulatory guidelines.

Communicate significant Comorbid findings, problems, changes in condition or environment to supervisors, the physician, and other team members involved with patient care. Initiate plans for patient safety using patient, family, and community resources. Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines.

Write physician orders to cover additional visits and changes to the Plan of Care, per agency policy.

Incorporate patient care goals into therapy care, evidenced by documentation in therapy notes.

Provide occupational therapy services in accordance with accepted standards of practice and certified by the patient’s physician.

Instruct patients and/or families on activities of daily living utilizingipe adaptive equipment as well asегь sensory integration and fine‑motor skills.

Assist patients and/or caregivers in securing necessary equipment, including fabrication of custom‑made splints.

Instruct patients and/or caregivers in the therapy home program, providing modification and clarification when necessary.

Consult and instruct Öl patient andcek family in meeting therapy and related needs.

Supervise or provide clinical direction to the Certified Occupational Therapy Assistant (COTA), Home Health Aide (HHA), and other clinical staff to ensure the therapy plan is followed and quality of care is provided.

Workiece Physical—demanding, high‑stress environment.

Perform full range of body motion including handling, lifting, and transferring patients.

Potentially work irregular hours, including call hours if applicable.

Required Qualifications

Graduate from an accredited Occupational Therapy program.

Current, unrestricted OT license valid for the state of work.

Minimum one (1) year of experience in a clinical care setting.

Able and willing to travel within branch/office coverage area.

Must have a dependable vehicle, valid driver’sхэг license, and current auto insurance in accordance with state laws.

Must be able and willing to travel 50% of the time.

Able to sit, stand, bend, lift, and move intermittently; able to lift 50‑100 lbs.

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
