Your Role We’re looking for an experienced and compassionate Home Health Physical Therapist to join our growing team. The right candidate will have strong clinical judgment, excellent communication skills, and a heart for patient‑centered care.
What You’ll Do
Conduct OASIS assessments and initial start‑of‑care visits to establish individualized plans of care
Evaluate functional limitations and develop personalized therapy goals and interventions
Provide lymphedema management and treatment, if certified
Guide patients through therapeutic exercises and mobility programs
Educate patients and caregivers on home exercise and safety techniques
Collaborate with physicians, nurses, and other members of the care team
Monitor patient progress, update care plans, and maintain thorough documentation
Ensure compliance with all Medicare and home health standards
What We’re Looking For
Current Florida Physical Therapist (PT) license (required)
OASIS and start‑of‑care experience (required)
Lymphedema certification (preferred but not required)
Previous home health experience strongly preferred
Excellent communication, time management, and documentation skills
A compassionate, dependable, and professional attitude
