Job Summary As a Licensed Practical Nurse (LPN) at Emerald Care Center Tulsa, you will play a crucial role in delivering high‑quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents.
This position offers flexibility with scheduling and is ideal for those looking to work on an as‑needed basis.
Work Environment Work is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided.
Responsibilities
Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
Monitor residents’ vital signs, document progress, and report any significant changes to the Charge Nurse or Director of Nursing.
Assist in the development and implementation of individualized care plans tailored to meet each resident’s needs.
Maintain clear and accurate documentation of patient care and communicate effectively with residents, families, and the healthcare team.
Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
Collaborate with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
Qualifications
Valid LPN/LVN license in Oklahoma.
Graduation from an accredited LPN/LVN program.
Previous experience in a skilled nursing facility or similar healthcare setting preferred.
Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and families.
Physical requirements: ability to lift and move patients, perform physical assessments, and stand for extended periods.
