Responsibilities

Provides direct clinical services in accordance with accepted standards of social/work clinical practice.

Plans and conducts individual, family and/or group therapy as assigned by the Director of Social Services or physician's order.

Conducts, assesses and interprets completion of Comprehensive Integrated Assessment.

Reviews all Multidisciplinary assessments and compiles a Multidisciplinary Treatment Plan under the supervision of the attending physician.

Performs case management and discharge planning as assigned by the Director of Social Services.

Attends multidisciplinary treatment team meetings, provides updates on assigned patients and their progress toward treatment goals; compiles reports into a multidisciplinary treatment integration summary to facilitate progress toward treatment plan goals.

Delivers patient services ethically, promoting respect, dignity and satisfaction; assesses and interprets patient responses to treatment with sensitivity to age, disability and culture.

Assesses and interprets age, disability and cultural needs; identifies safety, psychosocial, mobility and equipment needs; communicates appropriately to patients and families.

Conducts treatment and discharge planning in accordance with age/disability/cultural needs and involves the patient\'s support system as appropriate.

Performs other duties as assigned.

Requirements Educational / Experience Requirements: At a minimum a Clinical Social Worker (CSW), LMSW, LCSW, LMFT, LPC, LPC-A (TX Only) or Psychologist. 0 to 1 year post-graduate experience in clinical practice.

Qualifications / Skills

Must demonstrate knowledge to provide care appropriate to any age-related disability-related needs of the patients served.

Must demonstrate knowledge of growth and development principles relevant to the patient population.

Must assess and interpret data about the patient’s status to identify needs and provide appropriate care, including age-related care/disability/cultural sensitivity for adolescent, chronically mentally ill, adult, geriatric, chemically dependent, and dually diagnosed populations.

Must demonstrate in-depth knowledge of mental illness and chemical dependency and related family dynamics.

Must have basic knowledge of chronic mental illness.

Must have in-depth knowledge of clinical therapy modalities.

Excellent communication and group-process skills related to age-specific patient needs.

Ability to assess, intervene, treat and discharge planning.

Ability to pass the general age/disability post-test.

Versatile, flexible and able to perform in stressful situations.

Good time management and organizational skills.

Ability to coordinate multi-faceted workload.

Ability to work with individuals with mental/physical disabilities.

Must complete CPR certification and Oceans-approved de-escalation program.

Working Environment Subject to interruptions and high-paced, potentially stressful conditions; must cope mentally and physically.
